Insurance is an important part of your healthcare. Verifying coverage allows you to focus on your treatment and a hassle-free recovery. Please remember that the terms of your coverage is an agreement between you and your insurance company. Although we may participate with most of the plans within a particular insurance company there may be some we do not, or the coverage under each plan may vary. Please check with your insurance company regarding participation, coverage and pre-authorization prior to making an appointment. We will file up to two insurances (one primary and one secondary) for the patient to cover the cost of care. Any non-covered items, co-insurances and deductibles are your responsibility.
Please call the office to make an appointment in advance. If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care. We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
We make every effort to decrease the cost of your medical care. Therefore, we request payment arrangements for all office services at the time they are rendered unless prior arrangements have been made. We accept cash, checks, MasterCard, Visa, Discover and American Express for your convenience. If we are a participating provider of your insurance company, we will bill them. However, payment is the patient's responsibility. We will help in any way we can to assist you in handling claims.
Please request all prescriptions and authorizations for renewals between 9:00am – 5:00pm, Monday to Friday, when our full records are available. Renewals requested at other times will be filled only for extreme or emergent circumstances. Please do not wait until the last minute to request a refill. Normally, 48 hours notice is required.
Requests for copies of records must be made in person (or a form sent in) as HIPPA requires your original signature. Forms are available on our website. Adequate notice is required and records not being sent directly to another provider but instead requested for the patient's own records shall incur a processing charge (based on type and volume) which must be paid prior to release of the records.
Patients often carry personal disability insurance which pays the patient if they are out of work for an extended period of time. We are happy to help complete the medical portion of your claim form, however, there is a $10 fee for this additional service and adequate time to complete the form is required.
